[New Students] Smaller Schools Physical Education Registration

$150.00 per Student /month [4 Classes]

$40.00 per Class /student

* All Students: 10:00am - 11:00am [Ages: 6+]

Medical Release: I hereby authorize the adult staff of MHB to act on my behalf according to their best judgment in any emergency situation requiring medical attention and I hereby waive and release MHB and their staff and affiliates from any and all liability for any injuries or illnesses incurred while training. I have NO KNOWLEDGE of any physical impairment or otherwise that would prevent my child from participating, and according to our family physician my child is fit to participate in all training activities.

Likeness Waiver: By agreeing to this, I agree to allow any photos taken by MHB to be used in any marketing the company may undertake.

Liability Waiver: I/We, the undersigned, our heirs, executors and administrators, waive, release, and forever discharge MHB and its staff, from all rights and claims for damages, injury, or loss to person or property which may be sustained during participation in training activities. In agreeing to this Release the undersigned hereby acknowledges and represents that they are aware of the risks and hazards inherent in participating in this program, and that no insurance covering accident or injury has been provided for the participants, and that arrangements for any such insurance should be made individually by the undersigned.

Location: MHB Shooting Academy Indoor Facility is located on Thompson Boulevard at the rear of JCN (Jones Communication) which is the former Coca-Cola. Drive to the REAR of JCN through the gates on the Western Side next to Friendly Ford to the very rear of the property behind Bahamas Foam (Follow the ONE WAY sign around to the last Blue Warehouse at the Rear).
